- Complete an Application for Certified Verification of a Marriage Record.
- Submit a copy of your identification.
- Submit proof of your eligibility.
- Pay the non-refundable application fees.
- Submit your application to CDPHE.

Note: For a certified copy of the marriage certificate, civil union certificate or dissolution (divorce) decree, contact the county that issued the license or decree. The following documents are not accepted:
- Matricula Consular Card
- Novelty ID Card
- Non-expiring Identification Cards
- City or County Prison/Jail ID
- Souvenir birth certificates
- Temporary driver's license or temporary state ID card
If you cannot provide acceptable identification, it is suggested that you ask a spouse, parent, grandparent, sibling, or adult child, who can provide appropriate identification, to request the certificate. Proof of relationship is required, such as a birth certificate or marriage certificate.

- Ex-Spouses demonstrating direct and tangible interest
- Parents
- Stepparents
- Grandparents and Great Grandparents
- Siblings
- Children, Stepchildren,
- Grandchildren, and Great Grandchildren
- In-laws, aunts, uncles, nieces, nephews, cousins if they can demonstrate direct and tangible interest.
- Legal representative of any of the above
- Others who may demonstrate a direct and tangible interest when information is needed for determination or protection of a personal or property right.
- Paralegal
- Consular Consort
- Consulate offices
- Probate researchers with documented proof of a direct and tangible interest
- Creditors must present documentary evidence to establish direct and tangible interest
- Employer so benefits can be processed
- Insurance companies so claims/benefits can be processed

- The Office of the State Registrar has an index of marriage and dissolution (divorce) records for the years 1900-1939 and 1975 to present.
- For records issued between 1939 and 1974, contact the county that issued the license or decree.
- Civil union records are available from May 1, 2013 - present.

Need for the DocumentEdit
Marriage is a legal contract binding two people together, granting them rights and obligations. A Marriage Certificate is an official statement that two people are married.
A copy of the marriage certificate is often necessary when you apply for administrative procedures such as obtaining a passport or applying for certain social benefits.
The Colorado Department of Public Health and Environment - Vital Records Section provides official verification of marriage, civil unions or dissolution (divorce) recorded with the Office of the State Registrar of Vital Statistics.
A verification of marriage, civil union or dissolution (divorce) isn't the same as a certified copy of the certificate or court document. For a certified copy of the marriage certificate, civil union certificate or dissolution (divorce) decree, contact the county that issued the license or decree.
