Name Change

Go to a drivers license office and present one of the following documents needed to change your name on the records:

  • Certified marriage certificate issued in the U.S. (foreign marriage licenses are not accepted)
  • Certified court ordered name change document issued in the U.S.
  • Certified divorce decree issued in the U.S.
  • Certified original state or county issued birth certificate if returning name to maiden name
  • Certified tribal court marriage certificate with a file registration number and BIA seal indicating the tribal name
  • Certified civil union certificate issued in the U.S.
  • Immigration documents reflecting your new name


Address Change

Online:

  • After changing your address on-line, print your own label (for example, a return address label) with your new address and affix it to the back of your drivers license, permit or ID card

Mail In :

  • Complete the change of address form
  • Mail or deliver the form to a drivers license office
  • Print your own label (for example, a return address label) with your new address and affix it to the back of your drivers license, permit or ID card

In Person:

  • Complete the change of address form and leave in the drop box
  • Print your own label (for example, a return address label) with your new address and affix it to the back of your drivers license, permit or ID card

Note: You must purchase a new driver's license, permit or ID card if you want your new address displayed on the front of your card.

  • Emergency contact information may also be added to your record during in office issuance.

According to the Federal Real ID Act, they need to see proof of your current address. Bring one of the following that includes your name and address:

  • Utility bill
  • Credit card statement
  • Pay stub or earnings statement
  • Rent receipt
  • Telephone bill
  • Transcript or report card from an accredited school
  • Bank statement or checkbook
  • Mortgage document
  • Tax document
  • Homeowners/renter's insurance policy
  • Vehicle registration
  • Other items with address that can be reviewed by Driver's License personnel

For a minor child, a parent can provide the address with their license, as long as it is correct

InstructionsEdit

Name Change

Adult license and ID holders are required to renew their license/ID to change their name.

Minors, permit holders and CDL holders may be issued a duplicate.

If you were married outside of the U.S., please contact your county clerk and recorder's office for instructions on how to obtain a Colorado marriage certificate that can be used to change your name.

Please remember that all name changes must be on file with the Social Security Administration before visiting a drivers license office. Please wait 24 business hours for changes to reflect in the Social Security system.


Address Change

You may change your address on record by applying personally, online or by post.


Gender Change

You must visit a driver's license office for information and paperwork regarding gender change.


Updating Your Emergency Contact Person

You may update your emergency contact person online or in person.
