Procedure

The health certificate is a certificate issued by a qualified institution such as a CDC or a medical institution after the health check of the relevant item is carried out on the holder of the document. 

Apply in person:

1. First, you need to know the location of the health center in district or sub-district office to which you belong. For example, if you’re in Tianrun City, you should go to the Pukou Center for Disease Control and Prevention.
2. Gather the necessary documents you will require to make the application
3. Visit the health facility and present your application documents to the relevant authority.
4. Your documents will be verified and you will be issued with an application form to fill.
5. Fill in the application form with the correct information, attach your passport photo to it and submit it.
6. There after pay the medical fees for the examinations to commence, you will be required to undergo several medical check.
7. Once the medical checks are over you will be instructed on when to pick your results and Health certificate.
8. Foreigners can download the Foreigners Examination formbe filled by in a qualified institution in your residence country.
9. You will also be required to attach a recent passport photo stamped by the doctor or issuing authority

Instructions

You must have a physical health check before you can apply for a health certificate.

• Go to the relevant department to receive the medical examination form, paste a personal photo of the bare head on the medical examination form, fill in the basic information such as name, and pay the medical examination fee.

• According to the physical examination sequence, complete the physical examination items one by one, including the examination for blood collection, stool anal swab test, internal surgery routine physical examination, x-ray chest penetration and other checks.

• After the completion of the medical examination, it is necessary to carry out training on health laws and regulations; after passing the qualification, you will obtain the “Quality Certificate for Health Regulations Training”.

• If the medical examination results and training are all qualified, you can go to the relevant department to receive the health certificate.

Precautions

• Blood collection needs to be carried out under fasting conditions, the best time is before 9 am every day.

• Should avoid strenuous exercise before the physical examination, remember not to drink alcohol, cannot eat meat, so as not to affect the liver function test results.

• On the morning of the medical examination, fasting , with ID card, with one inch photo or card, pen .
• The day before the physical examination should pay attention to rest, avoid overeating, avoid drinking, avoid excessive fat diet, and do not stay up late, too tired. Pregnant women should not do X-ray chest fluoroscopy.
• Please take loose clothes during the physical examination to facilitate venous blood collection.
• After the blood can be eaten, please use a sterile cotton swab to press the local for 3 to 5 minutes, but cannot be paralyzed, the compression time is too short or hemorrhoids easily occur.
• Do not apply heat within 24 hours after blood draw.
