Apply In Person

  1. An application for a Permit or License to Remove Dead Body commonly known as conveyance or release of a corpse permit in Botswana shall be made to the Ministry of Health and Wellness offices. MOH Contact Details
  2. A Next of kin/family members to the deceased is required to submit all the supporting documentation as listed under the “Required Documents” of this page to the health officer where the deceased is been kept.
  3. Upon receipt of the complete dossier, the health officer will verify the documents authenticity, where satisfied he shall draw up the permit for release of the corpse and certify it before issuing it to the applicant immediately.
  4. No fees are charged for this service.

Required DocumentsEdit

  • Three certified copies of death certificate.
  • Three copies of non-infectious letter from the hospital.
  • Three certified copies of passport of deceased.
  • Three certified copies of passport of the accompanying person with the same surname (in case of different surnames, affidavit from police to be availed).
  • If person died at home, three copies of police affidavit needed.
  • Death certificate should always show cause of death, if not, affidavit from police to be availed to confirm there is no foul play.

  • Declaration by the applicant
