Procedure

1. Attend any Canberra Connect Shopfront or the Civic Driver License Service.
2. Show proof of identity.
3. Complete a Replacement/Cancellation form. Application for Replacement License [1]
4. Provide a statutory declaration stating that the driver license was lost, stolen or destroyed.
5. Pay the required fee.
