Apply through Phone Call
- You can apply by calling "14567" which is a National Helpline for the Elders and they should be able to facilitate you to get a Senior Citizen Certifiate or Identity Card.
Apply Online through Service Plus Portal
- Please use the following link to access portal : Service Plus
- Now click on the button "Know Your Eligibility"
- You will be directed to a new page. Now select "Direct Search" option on top right of the page.
- Now select State and type "Senior" in the search box and click "Go" button.
- Click on "Application for Senior Citizen Identity Card" link, this should take you to the application form.
- After filling the application form click on the "Submit" option on the bottom of the page to proceed further.
- Please follow the prompts to proceed with the submittion for the application.
Apply Online CM-Seva
- Please use the following link to access portal : CM-Seva
- Scroll down and in the search box type "Senior" and click "Search".
- You will provided with search result "Senior Citizen Certificate". Now click on "Access Service" option.
- This should take take you to new portal for making the application. Please follow the prompts and submit the application.
Apply through CSC:
- To apply for Senior Citizen Certificate, the applicant has to approach the nearest CSC : Link
- NOTE:To check the contact details of the CSC centre, please select the District and Block.
- The applicant has to visit the nearest centre.
- The applicant has to inform all the details to the respective operator.
- The operator will enter the details in the portal with the given details.
- The applicant apply through the operator.
- Please pay as per applicable service charges if any.
Apply In-Person
- To Apply for Delayed Registration (Late Entry) of Birth, the applicant has to approach the link “panchayat/ Directorate of Municipal Administration/Corporation / Health Department” .
- Also use the following link
- In the given page, please select the respective state as applicable to reach the respective state districts page. Please check and scroll down for the “Districts” section. Please click on the “View All” option to reach the districts page. Here click on the applicable district to reach the official district page.
- Here click on the menu and sub option as “DIRECTORY / PUBLIC UTILITIES / Municipalities” to reach a page with required contact information for locating the respective authorities office. Also Please look for other menu and sub options as “ABOUT DISTRICT / Who’s Who”.
- An application form will be provided to the applicant by the concerned officer.
- The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
- Make sure that the applicant has to attach all the necessary documents that are mentioned in the “Required Documents” section of this page.
- The applicant has to attach the photocopies of the required documents along with the application form while submitting.
- Submit all the documents with the completed form to the concerned officer.
- Fee for this procedure is mentioned in the “Fees” section of this page.
- After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
- An acknowledgment receipt will be provided. Please save it for future reference.
- Then the application will be processed further. All the updates on the application status will be sent via SMS to the applicant’s mobile number.
- Once the process and the birth record verification is completed with the birth occurred hospital, the applicant will get SMS from the office for collecting the certificate.
- The applicant has to visit the centre to get the certificate.

- Two photographs of the applicant.
- Proof of residence like Ration Card.
- Age Proof like Birth Certificate, Matriculation Certificate, School leaving certificate, Pension Payment Order if retired from service or any other age proof
