  1. Begin your application online. You may also download the form online by clicking this link.
  2. To complete the application process, you must visit a MVD office, submit all required documents, have a photo taken and pay for the ID card.


Note: If you are age 18 or under and unable to sign for yourself, a parent or legal guardian may sign the credential in the legal guardian section on the application.

  • Two forms of identification, at least one must contain a photo, or three forms of non-photo identification. See Identification Requirements for a listing of acceptable proof of identification. One proof must be listed under Primary.

EligibilityEdit

The identification card is available to all ages (including infants).


Note: You may not possess an identification card and a valid driver license at the same time.



FeesEdit

  • Ages 0-64: $12.00
  • Age 65 and older: no fee
  • Duplicate ID card: $12.00


Note: Those who are receiving Federal Supplemental Security Income disability checks may qualify for a free card based on eligibility.


Other Fees:

  • Returned check fee: $25.00



ValidityEdit

Arizona ID cards have no expiration date. However, you will be required to appear at your local Motor Vehicles Division (MVD) facility once every 12 years in order to have an updated photograph taken.

  • Applicant's Name
  • Residence street address (city,state,zip)
  • Mailing address (PO Box must be in the county you reside)
  • Address that you want to appear on your license
  • Gender
  • Weight
  • Height
  • Eye color
  • Hair
  • Date of birth


Note: You must provide your Social Security number, it will be used to verify your identity and to comply with federal and state child-support enforcement laws. It will not be used as your driver license or identification card number.



Need for the DocumentEdit

Most adults use their driver's license as their main form of identification. It's used to see Rated R movies, buy tobacco or alcohol, enter nightclubs and bars, board a plane, check into a hotel, cash a check, and verify a credit card. But what if you're too young to drive, you've lost your driving privileges, or you've just never had a license

For those who don't carry an Arizona driver's license but still need to prove their age or identity, an identification card is needed.
