Obtain a Certificate in Person

  • Same day service is not available for certified copies of certificates of birth resulting in stillbirth. Applications will be accepted and the certified copies will be mailed to the applicant. There is no pick-up service.
  • The office is open from 8:00 AM until 5:00 PM. If you are planning to go there in the afternoon, make sure you arrive before 4:00 PM.
  • Bring all necessary documents you will need to present or submit in order to obtain the certificate.
  • Appropriate fee. Acceptable payment methods are: cash, traveler's checks, cashier's checks, money orders, Visa, or MasterCard. Cashier's checks and money orders must be for the exact amount and made payable to Office of Vital Records.


Apply by Mail

  1. Complete the Application Form.
  2. Along with your application, you will also need to send a copy of the front and back of your valid government-issued picture identification which bears your signature or have your signature notarized.
  3. Do not forget to include your payment when you send your application. Acceptable payment methods are: cashier's checks, money orders, Visa, or MasterCard. PLEASE DO NOT SEND CASH. Cashier's checks and money orders must be for the exact amount and made payable to Office of Vital Records. If you pay by credit/debit card, you must include the full card number and expiration date on your application.

Note: An incomplete application will delay the processing of your request.

Maricopa County Office of Vital RegistrationEdit

In addition to the State Office of Vital Records, you can also obtain certified copies of Fetal Death Certificate from your local county health departments.

In Person:

  1. Fill out an application. Do not forget to sign your application.
  2. Be ready to provide your valid, government issued photo ID with signature. You must also bring any additional documents that may be required that establish eligibility such as proof of relationship and factual documents establishing legal or other vital interest.
  3. Pay the appropriate fee.


By Mail:

  1. Print and fill out an application for the certified copies of the certificate you wish to order. Do not forget to sign your application.
  2. Attach a clear photocopy of your valid, government issued photo ID with signature or have your signature notarized on the application form
  3. Do not forget to include any additional documents that may be required that establish eligibility such as proof of relationship and factual documents establishing legal or other vital interest.
  4. Make sure that you have included your payment in your application. Check the accepted form of payment, please remember, do not send cash payments through the mail.
  5. Mail your signed application, copy of accepted ID, factual documents and proof of eligibility, acceptable payment type and fee with a self-addressed stamped envelope to:

  6. Allow 1-2 weeks for processing to receive your certified copy in the mail.


Acceptable Identification:

  • Valid, Government Issued Photo ID with Signature:
  • Clear copies of FRONT and BACK of ID is REQUIRED
  • United States issued Drivers License or Identification Card
  • United States Territories Drivers License or Identification Card
  • United States Military Identification Card that contains bearer's signature (some do not)
  • Tribal Identification that contains bearer's signature
  • Passports United States or Foreign issued
  • Visa In a passport and the bearer's signature is on the passport not the Visa
  • Permanent Resident Card issued on or after May 11, 2010 which includes a signature
  • Resident Alien Card (Form I-551)
  • Employment Authorized Card (Forms I-766, I-688A or I-668B)
  • Temporary Resident Card (I-688)

Note: Expired, cracked, broken, taped or faded identification documents will not be accepted.

EligibilityEdit

In Arizona, a parent or a person authorized by a parent may apply for a certified copy of Stillbirth Certificates. If the individual making the application is not a parent, then the person authorized by a parent must provide a letter permitting the named party to apply on their behalf.
