Poland - Register a Foreign Birth

Procedure[edit]

  • Submitting an original certified birth certificate (extended or long form);
  • Ensuring that the original document:
  • Bears an Apostille stamp, where applicable (Annas comment I cant stress enough how important this Apostille bit is!!! without it your documents may turn out to be totally useless), or
  • Undergoes legalization by the Consul verifying the authenticity of the underlying documents;
  • Translating the document into Polish by a sworn translator;
  • Having the Polish consulate officially certify (urz?dowo po?wiadczone) the accuracy of each page of the translation;
  • Filling out the appropriate form (wniosek) and
  • Paying the associated fees (op?aty)

REGISTRATION OF A BIRTH CERTIFICATE OF AN ADULT[edit]

There are three ways to submit the application:

  • Personally - by submitting the following documents, in person, at the main Registry Office of the capital city Warsaw:
    • the original foreign birth certificate with apostille
    • a translation of the birth certificate
    • application to the Head of the Office of Registry Office of the capital city of Warsaw;
    • valid Polish passport or an original certificate of Polish citizenship OR presidential act granting Polish citizenship OR a decision act of restoration of Polish citizenship.
  • By means of persons living permanently in Poland - by sending the following documents, along with a certified authorization, issued to a person permanently residing in Poland, to register the birth certificate on behalf of the applicant, at Registry Office of the capital city of Warsaw.
    • the original foreign birth certificate with apostille
    • a translation of the birth certificate
    • application to the Head of the Office of Registry Office of the capital city of Warsaw;
    • valid Polish passport or an original certificate of Polish citizenship OR presidential act granting Polish citizenship OR a decision act of restoration of Polish citizenship.
    • an authorization in Polish, certified by a Polish Consul or public notary, to register the birth certificate on behalf of the applicant.
  • Through the Consular Section - by submitting the following documents personally at the Consular Section:
    • the original foreign birth certificate with apostille
    • a translation of the birth certificate
    • filled in application form to the Head of the Office of Registry Office of the capital city of Warsaw
    • valid Polish passport or an original certificate of Polish citizenship OR presidential act granting Polish citizenship OR a decision act of restoration of Polish citizenship;
    • valid foreign passport of the applicant.

REGISTRATION OF A BIRTH CERTIFICATE OF A MINOR[edit]

There are three ways to submit the application:

  • Personally - by submitting the following documents, in person, at the Polish Registry Office:
    • the original foreign birth certificate with apostille
    • a translation of the birth certificate
    • application to the Head of the Office of Registry Office;
    • valid Polish passport of at least one parent or an original certificate of Polish citizenship of at least one parent OR presidential act granting Polish citizenship which encloses minors;
    • original Polish marriage certificate of the parents or official statement that parents are not married.
  • By means of persons living permanently in Poland - by sending the following documents, along with a certified authorization, issued to a person permanently residing in Poland, to register the birth certificate on behalf of the applicant at the Registry Office.
    • the original foreign birth certificate with apostille
    • a translation of the birth certificate
    • application to the Head of the Office of Registry Office;
    • valid Polish passport of at least one parent or an original certificate of Polish citizenship of at least one parent OR presidential act granting Polish citizenship which encloses minors;
    • an authorization in Polish, certified by a Polish Consul or public notary, to register the birth certificate on behalf of the applicant;
    • original Polish marriage certificate of the parents or official statement that parents are not married.
  • Through the Consular Section - by submitting the following documents personally at the Consular Section:
    • the original foreign birth certificate with apostille
    • a translation of the birth certificate
    • filled in application form to the Head of the Office of Registry Office
    • valid Polish passport of at least one parent or an original certificate of Polish citizenship of at least one parent OR presidential act granting Polish citizenship which encloses minor;
    • original Polish marriage certificate of the parents or official statement that parents are not married;
    • valid foreign passports of mother and father of the minor;
    • valid foreign passport of the minor.

Instructions[edit]

Birth certificates of minors, born abroad, are registered on the basis of a written application, submitted by one of the parents to the Head of the Office of Registry Office, respective of one of the parents last legal permanent residence in Poland, along with the original birth certificate and its certified translation. If the parents have never had a legal permanent residence in Poland, the application must be submitted to the main Registry Office of the capital city Warsaw (m. st. Warszawy)

If the application is submitted by a person of 18 years or more who has never had a legal permanent residence in Poland, the application must be submitted to the main Registry Office of the capital city Warsaw (m. st. Warszawy)

In order to apply for Polish passport it is necessary to possess Polish birth certificates. People who were born or got married abroad can register their foreign birth certificates in Polish Registry Office through the Consular Section. As a result they will obtain Polish birth certificates which are necessary for obtaining Polish passport.

A copy of the application form (wniosek) to register a foreign birth or marriage certificate can be obtained from any consulate office or downloaded from most consulate websites.



Required Information[edit]

  • Name, gender and date and time of birth
  • Place of birth and gestational week of birth
  • Health status at birth, including length and weight
  • Mother's name, age, ethnicity and nationality
  • Mother's ID card number
  • Father's name, age, ethnicity and nationality
  • Father's ID card number
  • Location of birth (hospital, home etc.)
  • Name of the facility

Need for the Document[edit]

Birth certificates of adults or minors, born abroad, are registered on the basis of a written application, submitted by one of the parents to the Office of Vital Statistic (Urz?d Stanu Cywilnego), along with the original birth certificate and its certified translation.
