Procedure[edit]

When you call the DFA Appointment Hotline (02) 737-1000

  • All passport applicants scheduled for appearance must appear physically at the DFA at the date and time of their scheduled personal appearance for data and image capturing. This applies to all passport applicants, aging from 0 years old onwards. DFA is strict about the personal appearance system, absolutely no chaperone allowed for passport applicants aging 18 years old onwards.
  1. Your call will be received by a Customer Care Officer who will assist you in setting an appointment for your passport application.
  2. After getting your appointment schedule, you will be asked to provide your personal information for the passport application.
  3. You will be advised on the applicable requirements you need to bring for your passport application.
  4. You will receive a reference number relative to your appointment schedule.
  5. Go to passport.com.ph/print to print your application form using the Print Code given by the Customer Care Officer.
  6. On your appointment date, proceed to the DFA Consular Office located at ASEANA Business Park, Bradco Avenue corner Macapagal Boulevard, Paranaque City (see locator map). You must be at the DFA Consular Office 30 minutes before your scheduled time. Early/Late comers will not be entertained.
  7. Present your printed application form to the Appointment Counter.
  8. Get your queue number at the Information Counter and sit on the waiting section of the Processing Area. Wait for your queue number to appear on the monitor indicating what processing window you will go to.
  9. After processing, proceed to the Passport Enrollment Section located at the 2nd Floor. Pay the applicable passport processing fees (see passport fees).
  10. After payment, proceed to the Encoding Section for data capturing.
  11. After the Encoding Section, you can have your new passport released and delivered to your home without having to return to the DFA on the release date (optional). If availing of the delivery, proceed to the Delivery Counter to pay for delivery fee.

When you log in to the DFA Appointment Website passport.com.ph

  1. You will be asked to provide your personal information for the passport application on the online application form. Remember to fill in all the information slots correctly.
  2. Please take note of the requirements that you need to bring to the DFA Consular Office on your appointment date.
  3. Choose an appointment DATE and TIME.
  4. Check your email for your reference number and a link to print your application form. You will need to bring the printed application form to the DFA on your appointment.
  5. On your appointment date, proceed to the DFA Consular Office located at ASEANA Business Park, Bradco Avenue corner Macapagal Boulevard, Paranaque City (see locator map). You must be at the DFA Consular Office 30 minutes before your scheduled time. Early/Late comers will not be entertained.

Required Documents[edit]

GENERAL REQUIREMENTS

  • Personal Appearance
  • Confirmed appointment.
  • Duly accomplished application form may be downloaded from the DFA website, www.dfa.gov.ph
  • No need to submit a passport size photo.
  • Most recent expiring or expired passport
  • Valid picture IDs and supporting documents to prove identity.

ADDITIONAL REQUIREMENTS
Brown Passport or passports issued prior to 01 May 1995:

  • Bring original brown passport and photocopy of passport pages 1,2,3,4 and last page
  • Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR) and duly authenticated by NSO, to determine complete middle name. Transcribed Birth Certificate from the LCR is required when entries in NSO Birth Certificate are blurred or unreadable.
  • Report of Birth duly authenticated by NSO if born abroad

Green Passport or passports issued after 01 May 1995:

  • Bring original green passport and photocopy of first and last page of passport
  • Notarized Joint Birth Affidavit of Two Disinterested Persons
  • Any public document/s with correct full name, date and place of birth (i.e. Baptismal Certificate with readable dry seal or NCMF / OMA Certificate with photo and readable dry seal for Muslim applicants)

Machine Readable Passport (MRP) issued in the year 2007:

  • Bring original passport and photocopy of first and last page of passport
  • Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR) to determine complete place of birth. Transcribed Birth Certificate from the LCR is required when entries in NSO Birth Certificate are blurred or unreadable
  • Old cancelled passport may also be presented in the absence of an PSA Birth Certificate

Electronic Passport (E-passport)

  • Bring original e-passport and photocopy of first (data page) and last page of passport

Eligibility[edit]

Minors, Legal Age and Senior Citizens with a previous passport as long as you have all the requirements and documents needed.



Fees[edit]

  • Regular Processing: Php 950.00 (15 working days)
  • Rush Processing: Php 1,200.00 (7 working days)
  • An additional fee of Php 200.00 will be charged for processing of Lost Passports if it is still valid.



Validity[edit]

A passport is only valid for 5 years.

Instructions[edit]

  • You can renew or apply for a passport through a travel agency.
  • For your passport application, you can now enjoy the convenience of getting an appointed date, time, and option for passport delivery.
  • Requesting for new passport or passport renewal is now made accessible, all you have to do is provide your personal info online and choose an available date and time convenient for you. Applying for a Philippine passport is now efficient and world class.
  • Personal appearance is required for all applicants.
  • Confirmed appointment is required for all applicants except senior citizens, infants and minors below 1 year old, person with disability and OFWs. For OFWs, sufficient proof must be submitted such as a valid employment contract or OEC.
  • Earrings and contact lenses are not allowed during data capturing. Smiling with visible teeth is also not allowed.
  • Check all the data in the computer monitor and in the Enrolment Certificate before signing it.
  • Only immediate family members are allowed to get the passport in behalf of the applicant. Immediate family members include father, mother, brother, sister, spouse and children of legal age.
  • Passport shall be released to an immediate family member only with proper authorization letter. Passport of a minor applicant shall be released to parents only or to an authorized representative with Special Power of Attorney and Affidavit of Support and Consent.
  • Passports unclaimed after six (6) months will be cancelled per Department Order No. 37-03.
  • Check all data in the received e-passport upon release.
  • Number of processing days may vary depending on the location of the Regional Consular Office where the application was filed.
  • The Department may require additional supporting documents as may be necessary, especially for applicants with dual citizenship and with foreignsounding family names to prove citizenship.
