Procedure

The birth of a child shall be registered within thirty (30) days from the time of birth at the Local Civil Registry Office of the city/municipality where the birth occurred.

Eligibility

  • The parent/ guardian/ attendant at birth/hospital and clinic authorities, and;
  • The persons who have reached legal age but whose facts of births have not been reported at the Civil Registrar’s Office.

Fees

  • On time registration: Free
  • Delayed registration: Php100
  • Document stamp tax (if applicable): Php15

Validity

  • Birth Certificate Valid Forever

Instructions

Required Information

  • Complete name of the child (first, middle, last)
  • Complete name of the father
  • Complete maiden name of the mother
  • Date of birth (month, day, year)
  • Place of birth (city/municipality, province)

Need for the Document

A Birth Certificate is a vital record that establishes the birth of a child. It is an official form that gives details on the time and place of a person's birth, and his or her name, sex, mother's name and (usually) father's name. The word "birth certificate" can refer to either the original document or a certified true copy of or representation of the original record of birth document.
