Philippines - Obtain a Birth Certificate

Procedure[edit]


Please read the guide below to understand how to obtain a Birth Certificate in Philippines:

In-person[edit]

The Birth Certificate can be obtained in-person at either the PSA Serbilis centers or the SM Business Service Centers.

  1. Prepare the requirements:
    • One of the valid IDs as Postal ID, Student ID or library card signed by the school authority, the Current company/employee ID with signature of the employer or authorized representative, Driver’s license, Senior Citizen ID, NBI Clearance, Philippine passport, Police Clearance, SSS ID, GSIS ID, License issued by the Professional Regulatory Commission (PRC), or Integrated Bar of the Philippines (IBP) ID;
    • The Authorization letter of the birth certificate owner, your representative valid ID (if the applicant is your representative)

  2. In-person obtain the Birth Certificate:
    1. In PSA Serbilis centers
      Visit a suitable PSA Serbilis center Serbilis Directory and get the birth certificate application form (white) and your queue number from the Information Marshall. Complete the application form and submit it with the required documents at the Screener’s Desk. If the accuracy, consistency, and legibility of the application is approved, the screener will stamp your application form with “Okay for Payment”. Now you can pay the applicable fee at the Cashier and get the Official Receipt with a stamp containing the date and time of release. Please understand that how fast you receive the brith certificate will depend on whether the request is for a converted or unconverted document. Therefore, you can obtain your birth certificate within 2 hours after you submitted your application or take up to 10 working days of processing.
    2. In the SM Business Service Centers
      Visit the nearest SM Business Service Center Map and line up in front of the section that processes civil registry documents. Present the required documents and get the birth certificate application form then fill it out. Submit the application form and pay the applicable fee. After payment, you will be issued the Official Receipt and claim stub indicating when you can receive the request document. It may take 4 to 6 days for birth certificates to be processed and released at SM Business Service Center.

By Online[edit]

You can obtain the Birth Certificate by online through either e-Census Portal or PSA Helpline. The e-Census can deliver birth certificate in Philippines and anywhere in the world while the PSA Help delivers birth certificate only within the Philippines.

  1. Via e-Census
    Enter the Online Portal link and click the button “Click here to request now”, accept terms and conditions to go to the next page. Follow the its prompts to enter the requested information in the “Contact and Delivery Information” form; Choose the button “Birth certificate” to start making a request; Fill out the online application form; and make a payment either over the counter in designated banks or via online payment channels. Now you can wait for the delivery. How quickly the birth certificate will be delivered to you depends on your delivery address: 3-5 working days if you are in Metro Manila, 4-9 working day if you are in other cities/provinces in the Philippines and 6-8 weeks if you are in other countries.
  2. Via PSA Helpline
    Enter the PSA Helpline website link, click on the button “Order Now” at the lower left corner. Follow its prompts to choose Birth Certificate in the presented PSA certificates, choose the purpose of your request then fill out the PSA birth certificate application form. If your birth certificate was involved in a legal proceeding for adoption, legitimation, correction of entry, etc. in the past, choose the appropriate box from the available choices, if not, just select “None”. Enter the name of the requester (your name or your representative name will have different options). Enter your contact information and delivery address in the designated fields. If you are not available to receive the certificate, you can add up to 3 representatives who will receive it for you by presenting his/her valid ID as well as your valid ID and authorization letter. Perform your payment and wait for the document.


Note: PSAHelpline.ph was ordered to stop accepting applications from January 4, 2020 and onwards through a letter received from the office of the National Statistician of the Philippine Statistics Authority. To know more - link

A customer who still incorrectly dials the old seven-digit number will hear a special announcement saying the format has been changed to eight-digit, for a period of three months after the migration (March 18, 2019 to June 17, 2019).

Eligibility[edit]

  • Philippines citizen or his/her parent, spouse, direct descendant, legal guardian/institution-in-charge (if he/she is a minor), or his/her representative (must present the Authorization and owner’s ID together with his/her ID);
  • In addition, the following are also allowed by law to obtain a copy of the birth certificate on the person’s behalf: the nearest of kin, in case of the person’s death; the court or the proper public official, if the identities of the child’s parents or other details regarding his birth need to be verified during administrative, judicial, or other official proceedings

You are a Philippines citizen. Your Birth Certificate can be obtained by yourself or your parent, your spouse, your direct descendant, your legal guardian/institution-in-charge (if you are a minor), or your representative (must present the Authorization and your ID together with his/her ID). In addition, the following are also allowed by law to obtain a copy of the birth certificate on the person’s behalf: the nearest of kin, in case of the person’s death; the court or the proper public official, if the identities of the child’s parents or other details regarding his birth need to be verified during administrative, judicial, or other official proceedings. A Birth Certificate is a vital record that establishes the birth of a child. Need for the Document[edit]

A Birth Certificate is a vital record that establishes the birth of a child. It is an official form that gives details on the time and place of a person's birth, and his or her name, sex, mother's name and (usually) father's name. The word "birth certificate" can refer to either the original document or a certified true copy of or representation of the original record of birth document.
