Macau - Obtain a Certificate of Special Record for Minors

Procedure[edit]

Application in Person[edit]

  1. To obtain a Certificate of Special Record for Minors, visit a Macau SAR Identification Office. For applicants under the age of 16, their parents must be the one to submit the request for the certificate.
  2. Submit a completed application along with all the required documents.
  3. Pay the application fee.
  4. Your certificate will be ready in 5 working days. Express Service, 2 working days.

Application by Post[edit]

  1. For applicants who are outside Macao you may lodge your application by post.
  2. Access the Postal Application Service system of DSI to obtain an application number.
  3. Print out an application slip.
  4. Send the “Part 2” of the application slip and the required documents to DSI via mail. (Mailing address: Macau Postal Box No. 1089/ Avenida da Praia No. 804, Edificio China Plaza, 20 andar, Macau).
  5. After receiving the required documents, DSI will update the application progress in the “Postal Application Service” system. Applicants should check their application status in the system from time to time.
  6. When the application status changes to “Please pay for the application”,pay the fee to complete your application. You can settle the payment through these following methods:
    1. Pay online: by credit card in the “Postal Application Service” system.
    2. Payment made by an authorized person: An applicant can authorize a person in Macao to pay the fee at DSI in person on his/her behalf by stating it clearly in the application letter.

Note:

  • In case of having made a payment to the Macao Special Administrative Region Government mistakenly due to system failure or other special conditions, the person concerned should contact the Identification Services Bureau (DSI) immediately. DSI will return the relevant amount to the person concerned.
  • If the applicant wants to cancel the application and have the refund of the application fee after completing the transaction online, he/she has to make a written request to DSI for the refund.

Required Documents[edit]

If applicant is under the age of 16 , [submitted by his/her legal representative (parents or guardian)]:

  • Photocopy of the applicant's Macao SAR Resident Identity Card; for non-BIR holders, photocopy of applicant’s valid travel document or a valid document for entering Macao; and documentary proof of the connection between the applicant and Macao, such as Residence Permit, proof of schooling or proof of application for residing in Macao (e.g. a notice of Residence Permit issued by the Immigration Department of the Public Security Police Force or a notice of submitting the Certificate of Criminal Record issued by the Macao Trade and Investment Promotion Institute and so on)
  • A full set of applicant’s ten fingerprints (for first time application).
  • Original identity document of applicant's legal representative.


If applicant is or over the age of 16:

  • Application made in person:
    • Original Macao SAR Resident Identity Card (BIR); for non-BIR holders, present the photocopy of applicant’s valid travel document or valid document for entering Macao and documentary proof of the connection between the applicant and Macao.
  • Applications made by applicant’s parents or spouse
    • Photocopy of applicant's Macao SAR Resident Identity Card (BIR); for non-BIR holders: photocopy of applicant’s valid travel document or valid document for entering Macao and documentary proof of the connection between the applicant and Macao.
    • Full set of applicant’s ten fingerprints (for first time application).
    • Original identity document of applicant's direct relative;
    • Documentary proof of the kinship (if the relevant data is not archived at Identification Services Bureau);
    • Documentary proof of why the applicant cannot apply in person, such as medical record.


Application made by authorized person

  • Declaration (Samples can be obtained at DSI or from DSI website <www.dsi.gov.mo>) Forms Download- Declaration (Authorization for Certificate of Criminal Record Application). Any amendments, defacements or deletions made on the declaration should be affirmed by the declarant with a signature aside, and the signature should be identical with that on the identification document;
  • Photocopy of applicant's Macao SAR Resident Identity Card (BIR); for non-BIR holders: photocopy of applicant’s valid travel document or valid document for entering Macao and documentary proof of the connection between the applicant and Macao.
  • A full set of applicant’s ten fingerprints (for first time application).
  • Original identity document of the authorized person.


Application by Post

  • A duly completed application form or an application letter in which applicants should state clearly their name, parents’ name, date and place of birth, usage of the certificate, contact information and type of application (normal or urgent application). In the letter, the applicant should sign and impress a right index fingerprint next to their signature. For the first-time application, a set of ten fingerprint impression should be submitted to DSI (the form for fingerprint impression can be downloaded in DSI’s website);
  • Proof of residence in Macao or proof of application for residing in Macao, such as identification documents issued in Macao, proof issued by a school in Macao, a notification letter in regard to Residence Authorization application issued by the Immigration Department of the Public Security Police Force, a document issued by the Macao Trade and Investment Promotion Institute to request the applicant to submit the Certificate of Criminal Record and so on;
  • Photocopy of a valid travel document of the applicant;
  • If the applicant does not hold a Macao SAR Resident Identity Card and needs to state his/her parents’ name on the certificate, he/she should submit a photocopy of the relevant supporting documents, such as birth certificate. Such document is not required if the applicant has submitted it to DSI before.
  • If applicant is under the age of 16, the application must be lodged and signed by his/her legal representative. Photocopy of the legal representative’s valid travel document is also required.

Eligibility[edit]

  • Resident of the Macao SAR or has formerly resided in the Macao SAR;
  • Up to the age of 12 and under the age of 21;
  • If applicant is under the age of 16 , application must be made by his/her legal representative;
  • If applicant is or over the age of 16 , application could be made in person, or by his/her parents, spouse or an authorized person.

Fees[edit]

  • Standard Service: MOP50.00
  • Express Service: MOP150.00
  • Applicants who lacks financial capability due to the following conditions can apply for exemption of application fees:
    • Unemployment:
      1) Applicants have to write and sign a declaration to declare their unemployment status and they acknowledge that they will commit criminal offences for making false declaration.
      2) Submit the Employment Registration issued by the Labour Affairs Bureau (DSAL).
    • Poverty:
      1) The applicant has to write and sign a letter to request for fee exemption;
      2) Submit the original and a photocopy of a valid Beneficiary Card issued by the Social Welfare Bureau (IAS).
    • Disability:
      1) Applicants have to write and sign a declaration to declare their financial incapability;
      2) Submit the original and a photocopy of a valid Disability Assessment Registration Card issued by the Social Welfare Bureau (IAS).
    • Note: Additional supporting documents may be required for applicant’s financial incapability verification of DSI, if necessary.


Application by Post

Application fee + Postage (This Bureau will inform the applicant of the total amount in the “Postal Application Service” system of DSI)

  • Normal application: MOP50.00.
  • Urgent application: an additional fee of MOP150.00.
  • Postage: The amount may vary depending on the destinations and weight of the documents.

Validity[edit]

The Certificate of Special Record for Minors is valid for 90 days from the date of issue.
