  1. Download the declaration of voluntary surrender form. You can download the form here:
  2. Send the form along with your driver licence to DVLA:

For Bus, coach or lorry licences
  1. Download VOC99/CERT. You can download the form here:

It is applicable to those driver licence holders who cannot drive anymore. Those who don't meet the requirements for eligibility in having a driver licence.

  • When you decide to stop driving, you must tell the DVLA and send them your driving licence.
  • If you still want to drive but worried for your ability to do so, you can talk to your doctor.
  • You can also have your driving assessed in a confidential and objective test from the Royal Society for the Prevention of Accidents (RoSPA).

Need for the Document[edit]

There is no certain or legal age at which you must stop driving. The decision is yours to make. However, medical conditions can greatly affect your driving and might mean that you must stop and surrender your driver licence.
