Procedure[edit]
- You can request for a copy of your birth certificate in person or you may also send your request via email (see contact information below). Note: Currently there is no facility for you to apply on-line.
- When you submit your request make sure that you have provided the following details:
- your date of birth
- your place of birth
- the name of your parents
- your detailed current postal address if applied from overseas
- payment for the certificate. If applied from overseas payment at the Cook Islands High Commission/Consulate or via Western Union (for details please contact this office) NB. Payment must be in NZ currency
When Living Outside the Country[edit]
It may be possible for you to obtain a copy of your birth certificate from a Cook Islands overseas office or diplomatic representatives of the Cook Islands in foreign countries, you may use the link below under 'Office Locations & Contacts' section to see a list of addresses and contact details.

Eligibility[edit]
Who can apply for a birth certificate:
- You can apply for your own birth certificate.
- Close members of your family can apply for your birth certificate.
Fees[edit]
- For every certified copy of an entry in a register book kept in a Registrar's office, and for every certified copy of an entry in the office of the Registrar-General (when correct year is given): $25.00
- For every certificate of date of birth when correct year given, including any certificate issued pursuant to section 44 of the Act: $15.00

Instructions[edit]
The Registry Office only records births that occur within the Cook Islands. It does not record births of "Cook Islanders" that occur outside of the Cook Islands.
Required Information[edit]
You will need to provide the following information:
- your date of birth
- your place of birth
- the name of your parents
- your detailed current postal address if applied from overseas
Need for the Document[edit]
A Birth Certificate states the name and sex of a person, it also indicate the time and place of birth, it is issued after you have registered the birth of a child. A copy of the birth certificate is often needed when you apply for procedures, such as getting a passport, a driver license or a marriage license.
