- If you intend to possess a live eagle for more than a year for religious purposes, you must apply for a permission Form 3-200-78 (if necessary)
- Please submit a signed statement stating the specific issue or questions if your religious convictions forbid you from responding to any of the questions below
- A signed Certification of Enrollment attesting to the Principal Officer's membership in a tribe that has received federal recognition
- Include a signed authorization form from the person in charge of administering the property where the live eagle(s) will be gathered.

Need for the DocumentEdit
- In order to remove Bald Eagles or Golden Eagles from the wild for Native American religious purposes, a federal Native American Eagle Take authorization is necessary.
- This licence is only available to Tribes performing clearly historical religious rituals that call for the capture of live eagles from the wild.
