ProcedureEdit

Apply In-Person:

  1. The applicant will have to visit the nearest Service Quebec office, find the link for the addresses of the centres under the “Office Locations and Contacts” section of this page. Here is the contact Link
  2. Download and fill the application form with relevant details, use the following link to the application Form.
  3. The applicant will have to show proof of identity, find the required documents under the “Required Documents” section of this page.
  4. Submit the application form along with all supporting documents at the registry agent’s office.
  5. The applicant will have to pay fees, which is mentioned under the “Fees” section of this page.
  6. The applicant shall receive their birth certificate within 16 weeks from the submission of the application, certificate can be picked from the office or be sent by mail.

  1. To obtain a birth certificate, download and fill the application form with relevant details, use the following link to the application form Link
  2. Send the application form by mail to the following address: Directeur de l'état civiL 2535, boulevard Laurier Québec (Québec) G1V 5C6.
  3. Attach copies of documents mentioned under the “Required Documents” section of this page.
  4. The applicant will have to pay the fee, which is mentioned under the “Fees” section of this page.
  5. The fee-related details should be entered in the application form.
  6. The applicant shall receive their birth certificate by mail within 10 days of the submission of the application.

  • The applicant should have had their birth registered in Quebec.
