Procedure

Apply In-Person

2. To apply for a change of name in Prince Edward Island, the applicant needs to fill out the application form.
3. The forms can be found under the “Documents to use” section of this page.

5. Once filled, the documents need to be sent to the vital statistics office.
6. The applicant will need to visit the nearest Vital Statistics office.

9. Once approved, the applicant will receive the change of name document.

Eligibility

• The applicants need to reside in Prince Edward Island for at least 3 months.
• If the applicant is in the custody of a minor and wants to change their name, the minor should have lived in PEI for 3 months.
• A child 12 years or older must give consent to change their name.
