- To apply for a change of name in Prince Edward Island, the applicant needs to fill out the application form.
- The forms can be found under the “Documents to use” section of this page.

- Once filled, the documents need to be sent to the vital statistics office.
- The applicant will need to visit the nearest Vital Statistics office.

- The applicants need to reside in Prince Edward Island for at least 3 months.
- If the applicant is in the custody of a minor and wants to change their name, the minor should have lived in PEI for 3 months.
- A child 12 years or older must give consent to change their name.
