- To apply for a change of name in New Brunswick, the applicant will need to visit the nearest vital statistics office, find the address under the “Office locations and contacts” section of this page.

- The applicant will be given a form.
- Fill in the form with all relevant details.
- Submit the form at the office.
- The applicant will have to pay a fee.
- The form will be forwarded to the relevant authorities.
- The applicant will also have to get their fingerprint scanned.
- The applicant will receive their change of name document within 6 to 8 weeks.
Required DocumentsEdit
- Birth Certificates for those born outside Nova Scotia.
- proof of residence such as:
- Driver’s Licence.
- Health Card if the applicant was born outside of Nova Scotia.
- Marriage certificate if married outside Nova Scotia.
- Immigration documents.
- Any necessary consents from a spouse or other parent, or court order.
- A witness who can sign the documents.

EligibilityEdit
- The applicant can apply for their spouse or child if they are 16 or older.
- The applicant should have been born in Nova Scotia.
- If the applicant is born outside Nova Scotia, they must provide documents.
- The applicant has lived in Nova Scotia for at least 3 months immediately before applying for a name change.
FeesEdit
- Application fee(includes 1 Change of Name Certificate): $165.70
- There is no fee for those who are residential school survivors and their families.
- The applicant will have to pay a fee of 25$ for every additional name change application form.
ValidityEdit
The document is valid forever and does not need to be renewed.
