ProcedureEdit

Apply In-Person:

  1. To obtain a birth certificate, the applicant will have to download the application form and fill it. Here is the contact link to the NL digital page, in case the applicant has any queries: Link.
  2. Download and fill in the application form, use the following Application Form link.
  3. Complete the application form with relevant details.
  4. The applicant will have to make a fee payment, which is mentioned under the “Fees” section of this page.
  5. The applicant will need to carry original copies of documents as mentioned under the “Required Documents” section of this page.
  6. Submit the completed application form to the following address, found under the “Office location and contacts” section of this page.
  7. The applicant shall receive their certificate by mail within 8 to 12 weeks from the submission of the application.


Apply Online

  1. Visit the Vital Statistics website, here is the Link.
  2. Select certificate type as “Birth” and click on “Next”.
  3. Enter birth details and click on “Next”.
  4. Enter further details when prompted and click on “Next”.
  5. Once all the details have been correctly entered, make the fee payment using the card.
  6. Click on “Submit” and submit the application.
  7. The applicant shall receive their certificate within a few weeks from the submission of the application.


Apply by Mail

  1. Download and fill in the application form, use the following Application Form link.
  2. Attach original copies of ID documents as mentioned under the “Required Documents” section of this page.
  3. The applicant will have to send the completed application form to the following address: P.O. Box 8700 St. John’s, NL A1B 4J6.
  4. The applicant shall receive their certificate by mail within a few weeks from the submission of the application.

EligibilityEdit

  • The applicant must be 16 years of age.
  • The parent(s) of a child under 16 years of age. Parent(s) must be listed on the birth record or provide court documents proving parentage.
  • The parent(s) of a child over 16 years of age who is mentally incapable. The proof is required such as a court document or letter from a physician.
  • The applicant’s birth must have been registered in Newfoundland and Labrador.


FeesEdit

The applicant will have to pay a fee of 30$.

ValidityEdit

A birth certificate is valid forever and does not need to be renewed.

The applicant can order 2 types of birth certificates:

  • The short-form copy of a birth certificate includes;
    • Name of individual
    • Date of birth
    • Place of birth
    • Sex
    • Registration date
    • Registration number, and
    • Date issued.
  • The long-form includes;
    • Name of individual
    • Date of birth
    • Place of birth
    • Sex
    • Registration date
    • Registration number
    • Date issued, plus
    • The names, and the province or country of birth, of the parents.
