• You may apply in person at any of the following Government Service Centres or departmental offices.
• You can also apply by sending an application form to Vital Statistics.

Eligibility

Provide eligibility criteria's for going through this procedure.

Fees

There is no fee for a Death Certificate issued within the first year of death.

There is a $20.00 fee for any Death Certificate issued after 1 year from the date of death.

Validity

Explain the time until which the certificate/document is valid.
e.g. Birth Certificate Valid Forever
