ProcedureEdit

Apply In-Person:

  1. ToObtain Milk Bar Licenseis done in person by visiting the Agriculture and Food Authority Board Head Offices in Ghana.
  2. Address and contact details can be found in the following contact link
  3. Visit the office and get an application form for a Milk Bar Licenseas applicable to your requirement, from the respective department.
  4. Make sure that his/her have all the documents listed in the “Required Documents” section of this page.
  5. Payment of appropriate fees should be made and that can be found under the "Fees" section of this page.
  6. Now, complete the application form with appropriate information under relevant sections. Attest it with your signature once completed.
  7. Once completed filling the application form, submit it in person to the relevant authority at the office.
  8. Pay the necessary application fees and licensing fees for obtaining a Milk Bar License.
  9. After submission, the application and other documents will be forwarded for examination and verification processes.
  10. Once the verification and validation processes are over, the application will be approved and the officials will proceed to take required steps to issue the permit.
  11. When the document is ready to be collected, the applicant will receive a formal notification from the office and the applicant can collect it by any applicable means.
  12. New licenses take a maximum of 14 days to process.

  • Permit Fee -GHS. 2,500.00
