ProcedureEdit

Apply In-Person:

  1. Application for Survivor Benefits in Ethiopia is available in the Ethiopian Health Insurance Agency (EHIA) contact link
  2. Applicant must provide all necessary information in the application form.
  3. Documents that are needed for this procedure are mentioned under “Required Documents” section of this page.
  4. Applicant should carry photocopies and originals of all the documents while applying for this procedure.
  5. Applicant should attach photocopies of documents (whichever is necessary) along with the application form while submitting the application form.
  6. There is no fee for this procedure.
  7. If all above-mentioned procedures were followed properly by the applicant, the national social security will be provided within three months.
  8. Applicant can collect their document by visiting the office after three months.
  9. If there is no information related to survivor benefits is provided within three months, applicant has to visit the office to know status of their application.

  • Member Statement
  • Original Death Certificate of the deceased member
  • Original court decree where the member is presumed dead
  • ID of the Deceased person
  • Claimant’s ID card
  • ATM or Bank Card
  • Fund Transfer Authority Form
  • Claimant’s full set of fingerprints
  • Marriage Certificate or other evidence of marriage in case of the applicant being the deceased person’s spouse.
