- To Apply for National Tuberculosis Prevention and Control Program Card( Cartão do Programa Nacional de Prevenção e Controle da Tuberculose), the applicant has to approach the Ministério da Saúde (Ministry of Health) link
- An application form will be provided to the applicant by the concerned officer.
- The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
- Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
- The applicant has to attach the photocopies of the required documents along with the application form while submitting.
- Submit all the documents with the completed form to the concerned officer.
- There is no fee for this program card.
- Depending on the program's requirements, you might need to undergo medical assessments, tests, screenings, or consultations related to TB prevention and control.
- After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
- Then the application will be processed further. All the updates on the application status will be sent via SMS to the applicant’s mobile number.
- If your application is approved, you will receive instructions on how to obtain your "National Tuberculosis Prevention and Control Program Card." This card may grant you access to TB-related services, testing, and educational resources.
- The card might provide you with access to TB testing, treatment, and information related to TB prevention.
- This entire process can be completed within 2 – 3 weeks or as per the timeline advised by the respective department authority.
