Procedure

Apply In Person:

1. To Apply for National Health Support Program card for People with Cancer (Cartão do Programa Nacional de Apoio à Saúde da Pessoa com Câncer), the applicant has to approach the Ministério da Saúde (Ministry of Health) link
2. An application form will be provided to the applicant by the concerned officer.
