Apply In Person

  1. To Apply for National Cervical Cancer Control Program Card (Cartão do Programa Nacional de Controle do Câncer de Colo do Útero), the applicant has to approach the Ministério da Saúde (Ministry of Health) link
  2. Visit a designated healthcare facility that offers health services. Public health clinics and specialized treatment centers are typically involved in leprosy prevention and treatment.
  3. An application form will be provided to the applicant by the concerned officer.
  4. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
  5. Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
  6. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
  7. Submit all the documents with the completed form to the concerned officer.
  8. There is no fee for this program card.
  9. After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
  10. Then the application will be processed further.
  11. Schedule regular Pap smear screenings as recommended by healthcare professionals. Pap smears are key for detecting abnormal cervical cells early.
  12. If abnormalities are detected in Pap smear results, follow the recommendations of your healthcare provider for further diagnostic tests and evaluations.
  13. If necessary, discuss treatment options with your healthcare provider. Cervical cancer treatment might include surgery, radiation therapy, chemotherapy, or other interventions.
  14. Maintain any medical records, diagnostic reports, and treatment plans related to your cervical health.
  15. Regular follow-up appointments with your healthcare provider are essential for monitoring your condition and adjusting the treatment plan if necessary.
