Apply In Person

1. To Apply for HIV Infection Prevention and Control Program Card( Cartão do Programa de Prevenção e Controle da Infecção pelo HIV), the applicant has to approach the Local Health Care Facility or Ministério da Saúde (Ministry of Health) link
2. Based on the evaluation, a healthcare professional will develop a treatment plan that may include vision-related treatments, medications, or assistive devices.
3. An application form will be provided to the applicant by the concerned officer.

11. Once the process gets completed, the applicant gets a call from the office regarding the application.
