- To Apply for Hepatitis C Prevention and Control Program Card (Cartão do Programa de Prevenção e Controle da Hepatite C), the applicant has to approach the Ministério da Saúde (Ministry of Health) link
- Visit a designated healthcare facility that provides vaccinations and services related to hepatitis C prevention and control.
- An application form will be provided to the applicant by the concerned officer.
- The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
- Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
- The applicant has to attach the photocopies of the required documents along with the application form while submitting.
- Submit all the documents with the completed form to the concerned officer.
- There is no fee for this program card.
- After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
- Then the application will be processed further.
- If you haven't been vaccinated against hepatitis C, a healthcare professional will assess your eligibility and provide information about the hepatitis C vaccine. If needed, they will administer the vaccine according to the recommended schedule.

To be eligible to apply for the Hepatitis C Prevention and Control Program Card in Brazil if you meet the following criteria:
- The applicant should be a Brazilian citizen or permanent resident.
- The applicant should have a valid Brazilian identification document.
- The applicant should be at high risk of contracting hepatitis B, such as if you:
- Have had unprotected sex with an infected person
- Have shared needles or syringes with an infected person
- Have been born to an infected mother
