Apply In Person

  1. To Apply for Health and Prevention Project Card in Schools (SPE, Cartão do Projeto Saúde e Prevenção nas Escolas (SPE), the applicant has to approach the Local Health Care Facility or Ministério da Saúde (Ministry of Health) link
  2. Many schools in Brazil have health programs that focus on promoting healthy behaviors, providing health education, and addressing various health issues among students.
  3. These programs may include health education sessions, workshops, and activities that cover topics such as nutrition, physical activity, hygiene, mental health, substance abuse prevention, and sexual health education.
  4. Encourage your child to participate in any health and prevention programs or workshops offered by the school. These initiatives are designed to educate students about making healthy choices.
  5. An application form will be provided to the applicant by the concerned officer.
  6. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
  7. Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
  8. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
  9. Submit all the documents with the completed form to the concerned officer.
  10. The fee for health care program card is free of cost.
  11. After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
  12. Then the application will be processed further. All the updates on the application status will be sent via SMS to the applicant’s mobile number.
  13. Once the process gets completed, the applicant gets a call from the office regarding the application.
  14. If your application is approved, you will receive your Health and Prevention Project Card.
  15. This entire process can be completed within 2 – 3 weeks or as per the timeline advised by the respective department authority.
