1. To Apply for Basic Health Care Program Card for Persons with Chronic Kidney Disease( Cartão do Programa de Atenção Básica à Saúde da Pessoa Portadora de Doença Renal Crônica), the applicant has to approach the Local Health Care Facility or Ministério da Saúde (Ministry of Health) link

11. Then the application will be processed further. All the updates on the application status will be sent via SMS to the applicant’s mobile number.
12. Once the process gets completed, the applicant gets a call from the office regarding the application.
13. If your application is approved, you will receive your National Health Care Program Card for Persons with Kidney problem
