Proof of identification can be established by submitting a current (non-expired), legible photocopy of one of the following forms of identification:
- State Government issued Photo ID or Drivers License
- Bureau of Indian Affairs issued tribal ID card
- US Government issued Military ID card
- US Government issued Passport or Visa
- US Government issued Permanent Resident Card
If you do not have one of the prior forms of identification, you must provide a legible photocopy of two of the following:
- Social Security Card
- Utility bill with current address (can not be more than 3 months old)
- Bank statement with current address (can not be more than 3 months old)
- Pay stub (must include your name, SSN and the name and address of your employer)
- Car registration or title with current address (for the current registration year)
- IRS Tax Return (from the prior year)

EligibilityEdit
A certified copy of a birth record may only be requested by:
- The individual named on the record, provided they are at least 16 years old; or
- The mother named on the record; or
- The father named on the record.
If you are not one of these three individuals listed above, you may not request a certified copy of a birth record unless:
- You submit a court order requiring our office to issue a certified copy to you; or
- You submit other legal documentation listing you as an authorized representative of the individual named on the birth record; or
- You submit our notarized authorization form, signed by one of the three individuals listed above granting you permission to request a copy.
If you are requesting a birth record for an individual that is deceased, you must:
- Be an immediate relative of the deceased individual; (i.e. parent, child, grandparent, grandchild) or
- Be an authorized representative acting on behalf of the deceased individual; and
- Submit a certified copy of their death record if the death did not occur in North Dakota.
We must also require proof of identification of the person making the request before we can release a certified copy of a birth record.

InstructionsEdit
- The North Dakota Division of Vital Records (North Dakota Vital Records) issues certified copies of North Dakota birth certificates for events which occurred within the State of North Dakota.
- Birth Certificates are available for events that occurred within the State of North Dakota from 1930 to present.
- Effective January 1, 2008, the Division of Vital Records will need proof of identity for all individuals requesting a birth record. These new requirements are based on legislative changes that occurred during the 2007 North Dakota legislative session and can be found in North Dakota Century Code 23-02.1-27 (1).

- The certified copies will be sent by first class mail unless you specify and include the funds for special shipping through Federal Express for an additional $16.00 (add $6 for delivery to Alaska or Hawaii) or UPS for an additional $16.

Need for the DocumentEdit
You may need a birth certificate when enrolling in school, applying for a social security card or for certain legal proceedings. This can prove frustrating when you do not have a copy immediately on hand. If you want to obtain your birth certificate or if you would like to have an additional copy of your birth certificate you can obtain one with following the procedure given below.
