Bahamas - Obtain Lay-off (Claimant Card) Certificate or Termination of Service from National Insurance Board (Employee)

Apply Online:
- The terminated employee needs to approach the official National Insurance Board (NIS) website Link
- At the top of the homepage, go to the “Forms” section and click on the link which applies. Download form, print, complete and sign the form.
- Scan/take a photo of the fully completed and signed form.
- Also, scan/take a photo of your Termination of Services/ LayOff Certificate (green form).
- Email the scanned copy to: [email protected]
- Please ensure that your address, telephone numbers and your e-mail address are correct.
Note:
Alternatively, you can submit the completed form to a drop box located at the E. Humphrey Walcott Building, corner of Collymore Rock and Culloden Road, St. Michael.

EligibilityEdit
Terminated Employees can apply to get unemployment benefits from the NIS.
To qualify for the unemployment benefit, one must:
- have been insured for at least 52 weeks.
- have at least 20 contributions paid or credited in the 3 consecutive quarters ending with the quarter but one before that in which unemployment began.
- have at least 7 contributions paid or credited in the relevant quarter, which is one quarter preceding the contribution quarter in which the unemployment commenced.

Processing TimeEdit
Must be submitted within two (2) weeks of becoming unemployed. Failure to do so may result in the loss of a portion of the unemployment benefit.

InstructionsEdit
- To continue receiving the unemployment benefit, concerned persons are required to complete and submit the “Declaration and Re-employment” Form online. Form Link to Download and Link to Fill Online
- Once the person is re-employed, the claimant should write to the Director, National Insurance Office, stating that he/she was re-employed, found employment with any employer, name of employer or started self-employment.
- If the person continues to be unemployed, he/she must renew their registration for the unemployment benefit by completing and submitting the Declaration and Re-employment Form online every three (3) weeks as required.
- If anyone makes false statement or give false information for the purpose of obtaining the benefit, they may be prosecuted and liable on conviction to a fine of $1,000 or imprisonment for six months or both.
- If the person fails to submit a claim on the right day and the time, they may lose a portion of the unemployment benefit.
