Procedure

Apply In-Person:

1. To Obtain Proof of Age Card, the applicant has to approach Driver and Vehicle Services Centre or regional agent. Contact details are mentioned in “Office Locations & Contacts” of this page. Or use the following given Contact Link
2. An application form is available at the following Link
3. The applicant has to get the form in Access Canberra Service Centre and fill-in all the details in the form.
4. Applicant should make sure to attach all the documents that are mentioned under the “Required Documents” section of this page (Both photocopies and originals).
5. Submit the application with the required document to appropriate office.
6. Submitting address is mentioned in “Office Locations & Contacts” of this page.
7. Fee of this procedure has to be paid as per the authority’s request.
8. Proof of age card processed within 14 days.
9. Department of Transport will issue the certificate and notice the applicant.

• For Queenslanders who are 15 years of age or older, a photo identity card that serves as age verification is an acceptable alternative to a driver's licence or passport.
