Procedure

Apply In-Person:

1. To Obtain Proof of Age Card, the applicant has to approach a service centre. Contact details are mentioned in “Office Locations & Contacts” of this page. Or use the following given Contact Link
2. An application form is available at the following Link
3. The applicant has to get the form in Access Canberra Service Centre and fill-in all the details in the form.
4. Applicant should make sure to attach all the documents that are mentioned under the “Required Documents” section of this page (Both photocopies and originals).
5. Submit the application with the required document to appropriate office.
6. Submitting address is mentioned in “Office Locations & Contacts” of this page.
7. The fee of this procedure is mentioned under the “Fees” section of this page.
8. Proof of age card processed within 10 days.
9. Government of New South Wales will issue the certificate and notice the applicant.

• A 10 year NSW Photo Card may be given to qualified consumers who are 21 years of age or older.
