When registering a death, you will need to complete a registration form and provide the medical certificate of the cause of death, signed by a doctor to your local District Registration Office.
To register a death, you must bring a Death Notification Form stating the cause of death to any Registration office. You will obtain this from the doctor who attended the deceased during his/her last illness. You must then complete Part 2 of the Death Notification Form, this contains important information relating to the deceased person, for example, his or her date of birth and his/her occupation, the PPS number of the deceased person. You must then sign the Register in the presence of the Registrar. You will need to bring photo ID with you when registering the death.

EligibilityEdit
A death which occurs in Afghanistan can be registered by:
- any relative of the deceased who has knowledge of the details required to be registered (this includes a relative by marriage)
- a person present at the death
- a person taking care of the funeral arrangements
- the executor or administrator of the deceased's estate
- the governor, matron or chief officer of a public building where the death occurred
- a person living in and responsible for a house, lodgings or apartments where the death occurred
- a person finding, or a person taking charge, of the body
Most deaths are registered by a relative of the deceased. The registrar would normally only allow one of the other people listed above to do so if there are no relatives available or they cannot be traced.

Both births and deaths are required to be registered within six months.
The registration units are mostly located in the districts and large villages
Parents and relatives shall report all cases of death to the village heads or local authorities who shall report to the relevant registration bureau or municipality on special printed forms obtainable from the registration department. The forms shall be filled out in accordance with the instructions contained in the Regulations for Registration Bookkeeping, and shall be forwarded together with the passport of the deceased. When a deceased person has no parents or relatives, the village head or other local authorities shall be responsible for reporting the death.
The doctor who was responsible for the medical care of a person before their death or who examines the body after death, must complete and sign a medical certificate of cause of death. This certificate must be issued within 48 hours after the person's death and given to the funeral director. The doctor is not required to give notice of the cause of death of a person in circumstances in which the death is reportable to the Coroner.

Need for the DocumentEdit
The registration of the death is the formal record of the death. It is done by the Registrar of Births, Deaths and Marriages and you will find the address of the nearest register office in the telephone directory.
Deaths should be registered as soon as possible. You will require the written consent of a Superintendent Registrar to register any death that was not registered within one year of the date of death.
Information which might helpEdit
On certain occasions the Civil Registration Service are obliged to notify the Coroner of a death and depending on the Coroner's response they may wish to look at this in greater detail. If this occurs the Civil Registration Service will work in partnership with the relative and the Coroner's office.
There may be a delay where a postmortem is carried out, as you will not be able to get a medical certificate/Death Notification Form until the results are known. A death will be automatically registered where an inquest or postmortem was held by a Coroner and the Coroner's certificate has been received by the Registrar.
Other uses of the Document/CertificateEdit
When you are registering the death, you can get copies of the Death Certificate at the same time.

OthersEdit
If the death is subject to a coronial inquiry, the death will be registered without a cause of death. This is called an "incomplete" death registration. If required, a death certificate can be issued at this stage.
