Procedure

Request Driver records without Personal Information

1. Prepare a written request.
2. With that request include the following information: name,date of birth, driver license number of the record that you would like to request
3. Enclosed with it the necessary fee. You can attach a money order, personal or cashiers check.
4. Or even if you mailed your written request, you can go to the Motor Vehicle and Driver Licensing Division and make your payment there.
5. Include in your letter the exact address where you want the record to be delivered.
6. Send your letter request to the following address :

Request Drivers records with Personal Information

1. Submit a filled DOR -4681 ( Request from Record Holder )
2. All the pertinent information must be completed including the signatures of the record holder and notary public.
3. You can submit the form to your designated license office or you can mail it to the address below with the appropriate fees

1. If the requesting party is a business entity, you should submit a form DOR-4678 ( Request for MV/ DL records/ Security Access code ) for you to obtain a security access code given by the Department Revenue.

• Fax fee for transmitting a drivers record - $0.50
• Office processing fee - $2.00 : apply if purchased at a license office.

Need for the Document

Requesting for a Drivers record is divided into two : Records with Personal information and Records without Personal Information.

Records with Personal information will only be granted if the requesting individual have the express consent of the holder or is exempt under the Federal Driver's Privacy Protection Act ( DPPA). While Records without Personal Information which contain details about driving history, speeding tickets and other violation can obtain through a requesting partys written request.
