- Requesting a vital record without an application form
A record can be requested by writing to the Bureau of Vital Records at the above address and providing the following information:
- Full name of husband
- Full maiden name of wife
- Date of divorce
- Place of divorce issued
- Your relationship to groom or bride (specify which)
- Explain reason for requesting the statement

- Missouri Vital Records only issues copies of divorce statements. Missouri divorce records are maintained in the city/county where the divorce occurred.
- To request a certified copy of a divorce decree contact the Circuit Clerk (http://www.sos.mo.gov/MOroster/county_officials.pdf) in the county where the decree was granted.
- This agency only issues divorce statements for events that occurred from July 1948 to present. To request a copy of the certificate, please contact the county clerk's office in the county where the license was purchased.
- To request a divorce statement from a local health department, you may complete the application and submit it in person or by mail to the nearest local health department http://health.mo.gov/living/lpha/lphas.php
- Make check or money order payable to "The Missouri Department of Health and Senior Services". Checks must be drawn on a United States bank. A money order must be drawn on a United States bank or issued by the United States Postal Service. Do not send cash through mail.
- A Statement can also be requested by writing to the Bureau of Vital Records and providing necessary information
- When requesting a copy of a vital record without an application, a check or money order for the appropriate fee must be mailed to the Bureau of Vital Records
- Applicants must show identification when requesting certified copies of a vital record at the state health department.
- Mail-in requests must be notarized by an acceptable notary public.
