Procedure

Apply In Person

1. To Obtain Certificate of Medical Necessity, the applicant has to approach the Department of health and human services Centres for Medicare & Medicaid Services (CMS) link
2. The applicant has to download the form from the following link
3. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)

   Note: Form should be completed by both suppliers and physician.

4. Make sure that the applicant has to attach all the necessary documents that are mentioned in the “Required Documents” section of this page.
5. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
6. Submit all the necessary documents along with the completed form to the nearest CMS office.
7. The applicant has to pay the prescribed fee as requested by the authority.
8. After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
9. The applicant gets the notification through email from the officer for site – inspection.
10. After completing all the process successfully, the applicant gets the certificate by visiting the office.
11. The processing time for this procedure is up to 15 – 30 days.

Need for the Document

• A certificate of medical necessity (CMN) is documentation from a doctor which Medicare requires before it will cover certain durable medical equipment (DME). The CMN states the patient’s diagnosis, prognosis, reason for the equipment, and estimated duration of need.
