Apply In Person
- To Obtain Certificate of Medical Necessity, the applicant has to approach the Department of health and human services Centres for Medicare & Medicaid Services (CMS) link
- The applicant has to download the form from the following link
- The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
- Note: Form should be completed by both suppliers and physician.
- Make sure that the applicant has to attach all the necessary documents that are mentioned in the “Required Documents” section of this page.
- The applicant has to attach the photocopies of the required documents along with the application form while submitting.
- Submit all the necessary documents along with the completed form to the nearest CMS office.
- The applicant has to pay the prescribed fee as requested by the authority.
- After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
- The applicant gets the notification through email from the officer for site – inspection.
- After completing all the process successfully, the applicant gets the certificate by visiting the office.
- The processing time for this procedure is up to 15 – 30 days.

Need for the DocumentEdit
- A certificate of medical necessity (CMN) is documentation from a doctor which Medicare requires before it will cover certain durable medical equipment (DME). The CMN states the patient’s diagnosis, prognosis, reason for the equipment, and estimated duration of need.

- The medical necessity includes a requirement that the treatment is within the accepted standards in the medical community. This is defined in the health plan’s medical policy.
- A health plan must make its medical policy available to the applicant, if it is used to make a decision to deny the coverage.
