If your permit is lost or stolen, as a first step you would have to contact your local police station where you have to register a FIR which has to be submitted along with your application.

FIR Copy - A certificate to the effect that the permit was lost or non-traceable issued by the Station House Officer concerned should be submitted. If the permit is damaged one, then the remains can be submitted and FIR is not required.
