ProcedureEdit

The one and same certificate is required for an individual if he / she uses multiple names in different documents.
Apply In-Person:

  1. In order to apply for One and Same Certificate, the applicant has to approach their concerned revenue department office.
  2. Please go to the respective Village Officer’s / designated officer’s office in whose jurisdiction the candidate usually resides.
  3. Link for the office contact details: link Please select respective district button. User will reach respective district portal. Here select“ Directory” menu OR “DIRECTORY >Revenue Officers” OR “ABOUT DISTRICT > Who’s Who” OR “ABOUT DISTRICT > ADMINISTRATIVE SETUP > Wards and Panchayats” OR “DIRECTORY > PUBLICUTILITIES > Municipalities” as appropriate for the contact details.
  4. Go to the respective office. Consult with the designated authority about the applying procedure for this certificate. Collect the application form or write on a plain A4 sheet as advised.
  5. Complete the application form. Attach all the required documents as per our “Required Documents” section of this page and submit it to the designated receiving authority.
  6. After receiving the application, authorities will check the submitted application and document set.
  7. Please follow authorities guidelines.
  8. If all are in place the application will be accepted with applicable fees as appropriate.
  9. Authorities will make entries about this application in the manual record or electronically using computer.#Acknowledgement receipt will be given against every form received. So, collect it and please keep it safe for future reference.
  10. Applicant will be informed of the probable date of collecting the certificate.
  11. Designated authority will forward the application to the concerned Circle Officer / designated authority for his/her report.
  12. The Circle Officer / designated authority will verify the applicant’s details given in the application. Authorities will do local enquiry, facts check and scrutinize the records.
  13. Authorities will make a report after the successful verification and ground level due diligence of the application.
  14. This report will be sent back to the issuing authority.
  15. Based on the outcome of the above report and other internal processing, the issuing authority will make a decision.
  16. The status, viz. Approval or Rejection if any will be conveyed to the applicant through an SMS.
  17. If approved, the certificate will be hand delivered or sent through post to the address mentioned while applying or it will be issued as per applicable norms.


Apply through Akshaya

  1. Applicant must visit the nearest Akshaya centre. Link for centre: link
  2. Applicant shall obtain application form from the centre or download using the link
  3. Submit the completed application form, required documents and applicable fees to the centre staff.
  4. Submitted application will be processed and receipt with application number will be issued.
  5. Application will be processed further by the respective department. SMS triggered update on application status will be sent to applicant mobile.
  6. Upon notification applicant shall receive the certificate from where the application submitted.


Apply Online

  1. Use the following link to reach online portal link
  2. In the given page, please use the login credentials as appropriate under “Portal User Login” section by selecting “Portal User” and then click the “Login” button to login.
  3. If you are a new user, then user shall register using “New PortalUserCreation” link provided under “Portal User Login” from home page and complete the registration requirements, click the checkbox for the “I agree” declaration and hit “Validate” button to get a pop-up. Please click “Ok” button.
  4. Now, click the “Register” button to get the “OTP’s” in the registered Email and mobile number respectively.
  5. Please enter the received “OTP’s” at the appropriate spaces and click the “Confirm” button to register and use the login credentials to login.
  6. Once login, the applicant will reach the dashboard. Please select “Applicant Registration” option under “One Time Registration” on the left part of the screen to get “NEW REGISTRATION” page.
  7. Please complete the page prompts as appropriate and click “Check Duplicate” button. The applicant will get a pop-up. Please click “Ok” button to get the summary of your details. Please click “Confirm this is your Registration” button and the applicant will get a pop-up. Please click “Ok” button.
  8. In the dashboard please select “Apply For A Certificate” menu on the left side of the page to get a information screen, Go through the details and click “Get Started” button.
  9. Applicant will get a page to process. Please select your registration number from the dropdown list under “eDistrict Register No*”. Please select “One and the Same” from the dropdown list under “Certificate type” and select the other page requirements as appropriate.
  10. After the above selection process applicant will get online form page. Please fill in the required details as per prompt and click “Save” button at the bottom.
  11. Applicant will get a popup about “OTP” sent to their registered mobile. Please click “Ok” button.
  12. Please enter the received “OTP” at the prompted space against “Enter the One Time…” and click the “Submit Application” button to get a popup message “Your Application is saved successfully, Attach supporting docu…”. Please click “OK” button to reach “2.Attachments” section.
  13. Please upload the required documents as per prompt and go to the “3.payment” section. Please click the “Make Payment” button and pay the amount online as per your payments gateway option.
  14. Please pay using the applicable payment gateway option.
  15. Once all the above steps are completed, an acknowledgement will be provided. Please keep it safe for future reference.
  16. This application will be processed by the respective department as per how it will be processed in person.
  17. Applicant will get notification about the status on his registered mobile number provided while applying.
  18. Applicant shall download the certificate from the online portal where it was applied or it will be delivered as per applicable norms.


Note:
  • In some cases, applicant will be asked to meet the village officer or the designated authority in person.
  • Applicant has to bring original documents to the applying office while applying for cross verification purpose and scanning if required.
  • Without cross signature on photograph and signature on undertaking, the application will be rejected.
  • Certificate will be dispatched or handed over directly as per the request made while applying by the applicant. Mostly handover in office will be in practice to get counter signature from the applicant for the delivered certificate.
