Procedure

Apply Online through SevaSindhu Portal

1. Go to link and click on Departments & Services
2. Click on Planning Programme Monitoring & Statistics Department and select "Application for Death Certificate". Alternatively, you can search for "Application for Death Certificate" in the search option.
3. Click on "Apply online" button.
4. Enter the username, password/OTP, and captcha and click on Log In button
5. If you have the Registration No. of the previously issued certificate, select “Yes”. Enter the Registration Number and click on Search to get the details.
6. If don’t have the Registration No. of the previously issued certificate, select “No”. Enter the Date of Death, Name of Deceased, Father’s Name and Mother’s Name and click on Search to get the details.
7. Verify the details. If the details are correct, select the checkbox (“Yes”) and enter the No. of copies for Print. Enter the captcha and click on Submit to proceed further.
8. Verify all the details and click on Make Payment to proceed. Select the Mode of Payment and click on Make Payment to proceed
9. After payment is successful, an acknowledgment will be generated. Acknowledgment consists of applicant details, application details and payment details for the applicant’s reference.
10. To download the certificate, go to the link
11. Click on Track your application status. Select the option as appropriate and click on "Submit" button.
12. Check Current Status of the application. If it is delivered, Click on Delivered.
13. Under Issue Document(s), click on Output certificate
14. Death certificate will be downloaded. Click on print option to print the certificate

Apply Online through Digilocker

1. User shall use the following Link
2. In the given page, please click on the “Login / Register” button provided on the top right corner of the screen to reach the “Login or Create Account” page.
3. Enter your registered mobile number at the prompts as applicable and click on the “Continue” button to reach the “Verify OTP” screen.
4. Please enter the OTP at the prompt as appropriate to complete the OTP authentication process and click on the “Verify” button.
5. On successful verification, the dashboard page will be provided.
6. The page will display documents already issued under the “Your issued documents” section.
7. Please select the Death Certificate document type if issued already and it is listed to reach the corresponding page.
8. Please follow the page prompts as appropriate and follow the page requisite to download the Death certificate.
9. If not displayed under the “Your issued documents” section, then click on the “Drive” option to reach the drive page with used space information.
10. If the citizen already saved his or her document in the drive, please browse through the page for the document type to download.
11. If not saved in Disk, then click on the “Search” or “Search Documents” option or enter the certificate type in the “Search for documents” prompt and click on the search icon.
12. In the resulting page, please look for the “States” section and click your respective state as applicable.
13. In the given state centric page, please click on the certificate type as applicable.
14. The resulting page will display the certificate or the system will prompt the user to enter required details.
15. Please follow the page prompts as appropriate and follow the page requisite to download the certificate.

Apply through CSC

1. To Download or View Death Certificate, the applicant has to approach the nearest CSC link

   NOTE: To check the contact details of CSC centre, please select the District and Block.

2. The applicant has to visit the nearest centre.
3. The applicant has to inform all the details to the respective officer.
4. The officer will enter the details in the portal with the given details.
5. The applicant will get the downloaded certificate from the officer.
6. To Download or View the Death Certificate, the fee is free of cost.
7. The processing time for this procedure is within a minute.

Need for the Document

• A death certificate is either a legal document issued by a medical practitioner which states when a person died, or a document issued by a government civil registration office, that declares the date, location and cause of a person's death, as entered in an official register of deaths.

• Death certificate helps in closing bank accounts, claiming insurance, selling property of the deceased or any work related to banks. It is also required for property transfer or property tax payment. If the deceased is the owner of a property, then succession certificate will be required in the absence of a 'Will'.
