Procedure

Apply In-Person:

1. Application form for obtaining certificate of radioactivity is available in the following contact link
2. Applicant must provide all needed information in the application form.
3. Applicant should make sure to carry all documents whichever is necessary to the office while applying for this procedure. (Both photocopies and Originals).
4. Documents that are needed for completing this procedure are listed under “Required Documents” section of this page.
5. Applicant should make sure to attach photocopies of all documents that are needed for completing this procedure along with the application form.
6. Fee for this procedure is TZS 35,000.
7. If all these procedures were followed properly by the applicant, the registration will be done within 1 day.
8. Applicant can get their certificate on the same day of application.
