Apply In-Person:
- To apply for modification in your Identity Document or Identity book in person, the person has to visit any nearest office of the Department of Home Affairs.
- Please click on the following link for the contact details and address of the office of Department of Home affairs: Link Among the list of provinces, click on your province name and get the details accordingly.
- Go to the office of DHA and get the application for applying for modification in an identity document (Form BI-9) from the concerned department.
- Also, you have to get an application (Form BI-309) for providing the corrected information.
- Complete the application forms as required with relevant information. Make sure that you have all the documents listed in the “Required documents” section of this page.
- Once completed filling the application form, submit it along with all the required documents at the respective department in the office. Also pay the prompted fees at the office.
- The submitted forms and attached documents will be put into necessary verification processes by concerned authorities.
- Once when all the furnished information is found correct and the attached documents are found in order, the modification you requested for will be done accordingly.
