Apply In-Person:
- To make an application to change the place of birth in a birth certificate, the applicant has to file a petition to change the place of birth in their birth at the Local Office of the Civil Registrar Office of the place where the applicant was born.
- If you’re living abroad, you can file at the nearest Philippine Consulate.

- At the Local Civil Registry Offices, the applicant should approach the Customer Service Counter and collect the application form to file the petition for correction of the place of birth in the birth certificate.
- The petition for correction of the place of birth in the birth certificate shall be in the form of a notarized affidavit.
- The applicant will be required to submit all the documentary requirements together with the petition application to the Local Civil Registration Officer to prepare the petition. A list of the required documents can be found in the “Required Documents” section of this page.
- Upon submission of the application and required documents, the applicant should proceed to make payments for processing the petition. The application fees to be paid are mentioned in the “Fees” section of this page.
- The applicant should then present their payment receipt to the Local Civil Registration registrar, and wait for the Local Civil Registrar to prepare your Petition.
- The applicant should also secure a Notice of Publication of the change of place of birth in the local dailies.
- The law requires the publication of the petition at least once a week for two consecutive weeks in a newspaper of general circulation.
- Once the publication is done, the applicant should get the Proof of Publication and an Endorsement Letter from the Local Civil Registrar of Birth and then send the documents to the Office of the Civil Registrar General in Manila.
- Upon receiving the documents, the Civil registrar or Consul General will evaluate the petition. If it complies with the required format and contents, it will be posted in a public place for 10 consecutive days.
- After completing the posting or publication requirement, the Civil Registrar or Consul General will approve or deny the petition within five working days.
- Upon approval of the petition, the Civil Registrar or Consul General will approve the petition and send it back to the Local Civil Registrar for Certification.
- After certification of the petition the Civil Registration Officer will contact the applicant to collect the certified petition to change the place of birth in birth certificate.
- The applicant should then forward the certified petition to the Philippine Statistics Authority (PSA) for them to record the change and provide an Annotated Birth Certificate.
- The applicant can then collect their Annotated Birth Certificate from the PSA.
- The applicant will be contacted through a telephone call or SMS to collect their Annotated Birth Certificate when ready.
- On the date of collection the applicant will have to produce their proof of identification when collecting their birth certificate.
- The whole process takes a period of 21 days.

- A certified true copy of the birth certificate or of the page of the registry book containing the entry or entries you want to be corrected or changed
- A certification from appropriate law enforcement agencies showing that you have no pending case or no criminal record
- Not less than two (2) private or public documents upon which the correction shall be based like baptismal certificate, voters affidavit, employment record, GSIS/SSS record, medical record, business record, driver’s license, insurance, land titles, certificate of land transfer, bank passbook, NBI/police clearance, civil registry records of ascendants.
- Notice/Certificate of Posting.
- Payment receipt.
- Other documents which may be required by the concerned civil registrar.

- The following individuals are eligible to change the place of birth on the certificate:-
- owner of the record
- spouse of the owner of the birth certificate
- children of the owner of the birth certificate
- parents of the owner of the birth certificate
- brothers of the owner of the birth certificate
- sisters of the owner of the birth certificate
- grandparents of the owner of the birth certificate
- guardian of the owner of the birth certificate
- another person duly authorized by law or by the owner of the document sought to be corrected.
- if the owner of the record is a minor or physically or mentally incapacitated, the petition may be filed by his spouse, or any of his children, parents, brothers; sisters; grandparents, guardians, or persons duly authorized by law.

- A fee of one thousand Pesos (P1,000.00) as the filing fee.
- For petitions filed abroad a fee of $50.00 or equivalent value in local currency is charged.
