ProcedureEdit

Apply In-Person:

  1. To Change or Update Death Certificate Details, the applicant has to approach the Philippines Statistics Authority’s local civil registry offices link.
    Note: The applicant has to click Field office and select the region from the drop-down menu.
  2. An application form will be provided to the applicant by the concerned officer.
  3. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
  4. Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
  5. The applicant has to attach photocopies of the required documents along with the application form while submitting it.
  6. Submit all the documents with the completed form to the concerned officer.
  7. The applicant has to pay the prescribed fee as requested by the authority. The fee details are mentioned in “Fees” section of this page.
  8. After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
  9. An acknowledgment receipt will be provided. Please save it for future reference.
  10. Then the application will be processed further. All the updates on the application status will be sent via SMS to the applicant’s mobile number.
  11. This entire process can be completed within 7 days or as per the timeline advised by the respective department authority.

Required DocumentsEdit

  • A certified true copy of the death certificate or of the page of the registry book containing the entry or entries the applicant wants to be corrected or changed
  • At least two public or private documents showing the correct entry or entries, which will serve as the basis for the correction or change
  • A certification from appropriate law enforcement agencies showing that the applicant has no pending case or no criminal record
  • Other documents that the applicant, the city or municipal civil registrar, or consul general may consider relevant and necessary for the approval of the petition

EligibilityEdit

  • Any person who intends to Change or Update death Certificate Details is eligible to make this application.



FeesEdit

The city or municipal civil registrar or the consul general shall collect a reasonable fee for the processing of the petition. Here are the current rates:

  • P1,000 - for correction of clerical or typographical errors
  • P3,000 - for change of first name or nickname


If the applicant is filing with the consul general, the fees to be collected are as follows:

  • $50 - for correction of clerical or typographical errors
  • $150 - for change of first name or nickname


If the applicant an indigent petitioner, they exempted from paying the filing fee.

ValidityEdit

  • The Validity of the death certificate is forever.

  • A Death certificate is an official document setting forth particulars relating to a dead person, including the name of the individual, the date of birth and the date of death. When requesting for death certificate, the interested party shall provide the following information to facilitate verification and issuance of certification.
