Procedure

Online Appointment

1. The applicant has to make an appointment before vising the office using the following link
2. The applicant has to enter the following details to make and appointment:
   • Email Address
   • Name
   • Office, Company, Affiliation or Organization Name
   • Company / Business Address
   • Contact Number
   • Office to visit
   • Person to visit or transact with
   • Nature or purpose of visit
   • Date of visit
   • Time of visit
   • Other details on the appointment
3. After entering all the above-mentioned details, the applicant will receive and appointment schedule via email.
4. On the appointed date the applicant has to visit the office.

Apply In-Person:

1. To Apply for Safety Officer’s Permit or License, the applicant has to approach the Philippines Department of Energy nearby offices link
2. An application form will be provided to the applicant by the concerned officer.
3. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
4. Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
5. The applicant has to attach photocopies of the required documents along with the application form while submitting it.
6. Submit all the documents with the completed form to the concerned officer.
7. The applicant has to pay the prescribed fee as Mentioned in the “Fee” section of this page.
8. After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
9. All applications are processed within a reasonable time, having regard to the particular circumstances of each application, including the completeness of information and documents submitted to the authority by the applicant.
10. The authority may seek additional information from an applicant if this is necessary to assess the application. The authority does not need to deal further with an application until the applicant has provided this additional information.
11. Once everything is accepted the Authority will provide the certificate.
12. This entire process can be completed within 10 - 15 days or as per the timeline advised by the respective department authority.

Required Documents

• A duly accomplished and sworn application with emphasis on service record
• Endorsement from the RE Developer
• Two (2) pieces of 2" X 2" size pictures (any color) taken not less than three (3) months at the time of application.
• Proof of Payment of processing fee

Proof of qualification:
For duly licensed engineer

• Valid PRC license,
• Certificate of Employment showing at least two (2) years actual experience in occupational safety in the RE industry
• Basic Occupational Safety and Health (BOSH) Training Certificate

For Degree holder or two years in college

• Diploma or Transcript of Records
• Certificate of Employment showing at least five (5) years actual experience in occupational safety in the RE industry
• BOSH Training Certificate

For DOLE Safety Practitioner

• Valid Certificate of Accreditation.
