Procedure

Apply In-Person:

1. To apply for a Short Birth Certificate in person, the applicant has to visit the office of the National Population Commission of Nigeria.

3. Visit the office of the Commission and get the application for a Short Birth Certificate from the relevant department.
4. Make sure that you have all the documents listed in the “Required Documents” section of this page.
5. Now, start filling the form as required. Complete the application form as required with relevant details and information about the applicant and the child.
6. Once completed filling the application form, it should be attested with the applicant’s signature with date.
7. Attach all the documents with the application form and submit the application package to the relevant authority at the NPC office.
8. Please pay the fees recommended by the office. Then, the application will be forwarded to the respective authorities for verification and validation.
9. When all the information provided is found true and the documents are found in order, the concerned official will register the Birth, include the name in the register and issue an Short Birth Certificate to the applicant.

Eligibility

• Any person living in Nigeria, who needs to register his/her child’s birth, can apply.
• Parent(s), guardian or any other person legally responsible for the child is eligible to apply.
• The person applying must possess a valid proof of Identification.
• The applicant must possess a certificate of Birth as a proof issued at the Hospital/Health facility or an affidavit obtained from the Local Government Authority.

Instructions

• Applicants are advised to complete the application form neatly and completely with absolute legibility and appropriateness to avoid rejections. Also, all the required documents must be furnished completely.
• In case of birth took place in Hospitals/Health facility, Birth certificate issued at the place is required and for places other than hospital, an affidavit must be sworn before any Local Government Authority must be produced.
