ProcedureEdit

Step 1: Obtain a Police Report

  • You should start by visiting a police station to report the loss or damage of Kenya Enrolled Psychiatric Nurse (KEPsyN) Private Practitioner Registration Certificate. You are to acquire a police report from the police authorities which is a prerequisite to making the application.
  • Attach this police report with your application.


Step 2: Apply Online

  1. A licensed Kenya Enrolled Psychiatric Nurse (KEPsyN) Private Practitioner can replace their lost or damaged Kenya Enrolled Psychiatric Nurse (KEPsyN) Private Practitioner Registration Certificate online by visiting the Nursing Councils of Kenya Online Services Portal through the following link: NCK Online Service Portal
  2. Log into theNCK 'Online Service Portal using your “Username or Email address” and your password.
  3. Upon entering your accounts’ homepage, click on the “Replacement” tab in the menu on the left pane to enter the next page.
  4. On the Replacement page , click on the “Make Replacement Application” button to enter the next page where you are required to provide information for your application.
  5. Please provide required information for registration. These include:-
    • Email
    • Mobile
    • Practice type (Enrolled)
    • Area of practice (Select Psychiatry)
    • Work Station Type
    • Work Station /Facility
    • Select Employer.
    • Workstation /Facility
    • Please attach the police report obtained
    • Attach copy of damaged/lost private practice registration certificate if available.
  6. Confirm that all the information you have given is correct and click on the “Apply for Replacement ” button.
    • This will cause an invoice for payment of the replacement application fees to be generated.
  7. On the Invoice page click on the “Click to Pay Now” button to pay for the Replaced Kenya Enrolled Psychiatric Nurse (KEPsyN) Private Practitioner Registration Certificate.
    • Proceed to make the payment using the instructions given online. The Payment instructions will be generated and displayed automatically in the portal. More information about the replacement fee to be paid is given in the “Fee” section of this page
  8. Upon making the application payment, a payment receipt and the Replaced Kenya Enrolled Psychiatric Nurse (KEPsyN) Private Practitioner Registration Certificate will be generated online.
    • Note: Wait for approximately 10 minutes then refresh the invoice page. After refreshing the page, you should find copies of the receipt and Private Practitioner Registration Certificate ready to be printed.
  9. Click on the “Download Receipt” button and the “Download Certificate” button to download the payment receipt and Replaced Kenya Enrolled Psychiatric Nurse (KEPsyN) Private Practitioner Registration Certificate respectively.
    • Print the receipt and Registration Certificate and keep them for reference.

Note

  • A nurse issued with a registration certificate to engage in private practice under the Nurses Act cap 257 shall do the following—
  1. Write and display his name and qualifications in an unostentatious manner and in accordance with the code of professional conduct applicable to nurses:
    • Provided that a nurse shall not use any word to imply that a private clinic or a medical centre is a hospital or a nursing home
  2. Keep in his private clinic adequate stock of essential drugs and maintain an accurate record of all drugs to which the Pharmacy and Poisons Act (Cap. 244) applies
  3. Immediately notify the Medical Officer of Health in his area of practice any notifiable disease which has been treated at his clinic
  4. Where death occurs in his clinic, inform the Medical Officer of Health in his area of practice and the Officer Commanding Police Division in that area, without delay.
