#When you turn 80 years of age and every year thereafter, you will receive a Medical Assessment Certificate - Senior driver&amp;#039;s license renewal declaration (form M108A) in the post, approximately 12 weeks before your driver&amp;#039;s license is due to expire.&lt;br /&gt;

#Fill in the front page of the form.&lt;br /&gt;
#Make an appointment with your regular medical practitioner (doctor) or health professional (specialist).&lt;br /&gt;
#Take the form and any aids you have with you to your appointment (e.g. hearing aids, spectacles).&lt;br /&gt;
#Your medical practitioner (doctor) will complete the rest of the form and submit it to us by fax, mail or scanned email. The fax service is available when a driver&amp;#039;s license imminent. GPs can fax an M108A to the Department of Transport when urgent.&lt;br /&gt;
#Your medical practitioner&amp;#039;s recommendations will be reviewed and consider other factors such as your previous driving history, demerit points incurred and your previous medical information.&lt;br /&gt;
#If the Department is satisfied that you are fit to drive, your license will be renewed or, in certain circumstances, a renewal will be posted out. If you are deemed unfit to drive, you will be notified in writing. You can appeal the decision  refer to the last page of the form for more information.&lt;br /&gt;

#Read the publication How to pass your driving assessment (booklet) - Senior&amp;#039;s Edition&amp;#039; to familiarize yourself with how the assessment works and what you will be assessed on. The &amp;#039;Drive Safe handbook&amp;#039; is another useful publication as it provides information on Western Australia&amp;#039;s road rules.&lt;br /&gt;
#Book a free* Practical Driving Assessment. Bookings can be made:&amp;lt;br&amp;gt;online: http://www.transport.wa.gov.au/LandingPage.asp?pda;&amp;lt;br&amp;gt;in person at a Driver and Vehicle Services Centre or Agent; or&amp;lt;br&amp;gt;by telephone on 13 11 56. Bookings are free of charge for senior license renewals (Drivers who are 85 or over).&lt;br /&gt;
#Undertake the Practical Driving Assessment.&lt;br /&gt;
#Your PDA results along with your medical practitioners recommendations will be reviewed and consider other factors such as your previous driving history, demerit points incurred and your previous medical information.&lt;br /&gt;
