*If you are applying for a certificate or a change of name or a correction, you must provide at least three forms of identification:&lt;br /&gt;
**ONE document from each list (1, 2 and 3).  At least one containing a photograph; or&lt;br /&gt;
**ONE from List 1 and TWO from List 2.  At least one containing a photograph; or&lt;br /&gt;
**TWO from List 2 and ONE from List 3.  At least one containing a signature.&lt;br /&gt;
*All forms of identification produced MUST be current.&lt;br /&gt;
*Documents from List 3 MUST show your current residential address.&lt;br /&gt;
*Bank statements, utility accounts or rates notices MUST have been issued within the last six months.&lt;br /&gt;

*Australian driver&amp;#039;s licence (with photo)&lt;br /&gt;
*Australian passport (with photo)&lt;br /&gt;
*Australian firearm&amp;#039;s licence (with photo)&lt;br /&gt;
*Defence force/Police ID card (with photo)&lt;br /&gt;
*Department of Immigration and Citizenship (DIAC) certificate with of evidence of residence status (with photo)&lt;br /&gt;
*Over 18 or Proof of Age identity card (with photo)&lt;br /&gt;

List 2&lt;br /&gt;
*Debit or Credit card (one or the other, not both) issued by a financial institution&lt;br /&gt;
*Document of Identity issued by the Passport Office&lt;br /&gt;
*Entitlement card issued by the Commonwealth or State Government (Centrelink, Health Care card, Veterans Affairs card etc)&lt;br /&gt;
*Full birth certificate issued in Australia (birth extracts not accepted)&lt;br /&gt;
*Medicare card&lt;br /&gt;
*Naturalisation, citizenship or immigration papers issued by DIAC&lt;br /&gt;
*Overseas passport with current Australian Entry Permit&lt;br /&gt;
*Security guard or crowd control licence (Australian)&lt;br /&gt;

Applying for certificates on behalf of another person&lt;br /&gt;
*If you are applying for a certificate on behalf of another person, you must provide the following:&lt;br /&gt;

Payment Options&lt;br /&gt;
*The following payment methods are accepted at registry:&lt;br /&gt;
**Credit card - MasterCard or Visa only and card number must have 16 digits. &lt;br /&gt;
**Available at the Perth Registry and selected Western Australian courts&lt;br /&gt;
**No other credit cards or travellers cheques will be accepted&lt;br /&gt;
**Eftpos (debit card)  when paying in person at the Perth Registry or at selected Western Australian courts.&lt;br /&gt;
**Cheque or money order - made payable to: Registrar of Births, Deaths and Marriages.&lt;br /&gt;

*If information on a certificate issued by the Western Australian Registry of Births, Deaths &amp;amp; Marriages is incorrect or incomplete, you should contact the Registry.&lt;br /&gt;
*Where the error was made by the Registry, no fee is payable and any certificates issued will be replaced free of charge.&lt;br /&gt;
*However, if incorrect or incomplete information was provided to the Registry, then a correction fee is payable. This fee includes the issue of a replacement certificate.&lt;br /&gt;
*Any request to correct certificate information must be supported by documentary evidence which confirms the correct details.&lt;br /&gt;
*Certificates will only be corrected where:&lt;br /&gt;
**you are an eligible applicant;&lt;br /&gt;
**suitable identification is provided; and&lt;br /&gt;
**fees are paid.&lt;br /&gt;
*Note: If your certificate was issued interstate, contact the relevant Registry office.&lt;br /&gt;
*To correct Registry records, applicants need to supply identification to verify they are entitled to correct the registration.&lt;br /&gt;
*If applying for a certificate in person three forms of your original identification documents or clear photocopied and certified identification documents must be provided.&lt;br /&gt;
